Critique Article: Breast Feeding Essay

Breast feeding has been known from ages past to be an incomparable method of providing the best food for the healthy development and growth of new born babies. It also has a distinctive emotional, psychological and biological impact on both the mother and baby. Breast feeding is less expensive and has been traced to help in decreasing the risk of infection ,asthma,obesity,type 1 & 2 diabetes, leukemia, SIDs, necrotizing enterocolities (NEC), mortality and morbidity rate in children . In mothers breastfeeding can help in reducing the risk of type 2 diabetes, postpartum depression, ovarian, breast and other forms of cancer. Regardless of all the benefits of breast feeding, the rate and duration has been consistently low worldwide due to some cultural, economic, social and professional reasons (WHO, 2013). Problem Statement The author noted a clinical problem in Lebanon, which is increase mortality and morbidity rate as high as 27/1000 births. The death that occurs in children under the age of 5 is associated with poor nutrition and health status of mother and babies. The breastfeeding initiation rate has been inadequately low and varies between 63. 8% and 96% . Mother who exclusively breasted for less than one month were 58. 3% and 6 months old infant were 10. 1% – 4. 1% and only about 27. 1% breastfed for one year. More importantly the author noted that there was no government organization to promote and support breastfeeding mothers, and postpartum mothers are only entitled to forty (40) days maternity leave. The author attributed a lot of predisposing factors to the low prevalence and duration of breast feeding, giving birth by caesarian section, low socio-economic status, religion of parents, and having a pediatrician who is a male. Some hospital practices like separation of babies from mothers, introduction of formula, water, sucrose at birth instead of breast milk. Most mothers look at breastfeeding as torture to be tolerated for the sake of their newborn baby. Possibly, if mothers are aware of the benefits of breast feeding for their own health, they will be less ikely to leave when they run into problems. The author noted that all these factors have been previously cross-sectioned except exploring mother’s experiences or perception towards breastfeeding. Purpose of this Study The purpose of this study is to use qualitative method to identify barriers, promote and support breast feeding among Lebanon women, and explore their perception and experience during breastfeeding. Another purpose was to encourage health workers to provide proper information and also have a good understanding of management of breastfeeding. Some of the question that was design to be answer was ‘Tell me about your previous experiences with breast feeding? Tell me what motivated you to want to breast feed? Tell me about your perception on breastfeeding your baby? Tell me how long you intend to breast feed? The research questions were answered by the participants during the interview and they were related to the purpose of the study (Nabulsi, M. , 2011). Literature Review This research study was approved by the â€Å"Institutional Review Board at AUBMC, and hospital administrators of HDF and SGH†. The author conducted this research using qualitative method to explore postpartum mother’s experience; feelings and perception about breast feeding. The participants were recruited using theoretical sampling in the three different hospitals and they all had a full term delivery of healthy babies. The first interview was done by the researcher using focus group discussion with 5 open-ended questions between November and December 2007. The second section was conducted by the researcher with 10 postpartum mothers who were shared into group of 3 to 4 in each group. This interview was done in â€Å"Arabic† language for about 60 to 90 minutes and it was recorded in tape and was transcribed. The second stage was done with in-depth interview with 36 mothers between January and May 2008 with 12 members from AUBMC, 10 from HDF, and 14 from SGH. The researcher followed them up for up to one year or until breastfeeding was stopped. Reference point interviews were done with each of the participants for about 30 to 60 minutes which were recorded and audio-tape before they were discharge from the hospital. The follow-up interviews were done by telephone and face to face in the participants home every month for the first 6 months, then once every other month for another 6 months at each time the interview lasted for about 10 to 30 minutes. These follow up were done to discover breastfeeding barriers, mother’s experiences, perception, feelings and natural factors that may interfere with their decision to successfully breastfeed. They were all asked about their current status with breast-feeding and the reason for still breastfeeding or stopping if they did. According to the author in spite of their knowledge of breast feeding benefits, most of the participants also expressed some personal fear of excessive weight gain, change in shape of their breast, cultural belief that their milk is bad or could cause harm to the baby, breast feeding is so tiring and exhausting, pain, not enough breast milk, infant constantly crying, sleep deprivation. The author concluded that negative experiences during breast feeding seemed to have a significant impact on mother’s decision to stop breastfeeding( Nabulsi, M. , 2011). The author cited about 22 relevant journal articles and literature review and only about three were less than five years which is very normal in a qualitative research. There was no adequate information in the literature review to build a logical augment and no weakness was indicated during the study. The perspective from which the study was developed is from the women of Lebanon who were consistently experiencing low breast feeding rate and the author did not use grounded theory qualitative inquiry, and no diagram or framework was developed from this study. In conclusion breast feeding is still the most natural and acceptable food for babies worldwide. Making an attempt to increase the initiation and duration of breast feeding without significant change in the feelings and perception of mothers towards breast feeding may result in more mothers having negative experiences and consequently reduces the rate of breastfeeding. There will be great need for health professional to prepare pregnant mothers on how to successfully breastfeed and this will help improve their confidence and willpower (McInnes, R. Chambers, J. , 2008). More education will need to be done with certain population including low-income mothers, ethnic and cultural group and educating nurses and doctors in breastfeeding management. Finally there will be need to develop government policies and program that will assist mothers that are breastfeeding, extending postpartum leave, providing child care at work places for mothers. However in Lebanon further research is required to assess the efficiency of future interventions in regards to breast feeding.

What is an Epidemic? Well there are different kinds of epidemics. Epidemics can be Products, diseases, Trend’s, and or Crimes. In order for an epidemic to tip, there are three rules that apply for an epidemic to tip. Epidemics are the phenomena word of mouth, or any number of mysterious changes that mark everyday life is for us to think of them as just that.Epidemics are a contagious behavior; the rules in an epidemic are the Law of the Few, The Stickiness Factor, and the Power of Context. The Stickiness Factor The stickiness factor is some kind of information that makes it memorable and sticky, basically something that makes a person remember easily or a message that makes an impact. One of the three Laws that applies too, Toms Shoes is the stickiness of the shoes. The Epidemic that crossed my mind was TOM’s shoes. The co-founder of TOM shoes was Blake Mckoskie, his name was not Tom, Blake’s chose toms because it stands for Tomorrows Future.Back In 2006, Blake went to Argentina, and discovered that the kids there had no shoes, So he came up with the Idea that for every pair of shoes purchased, he would give another pair to a child in need. Tom’s shoes missions is the One for One deal, which I stated above. The reason why Toms tipped was because of the One for One. This is what made TOMS so sticky, just the other day I wore a pair of Toms and this lady stopped me and somehow noticed my shoes and said to me, â€Å"Are those the shoes? † where they give a pair for free? and she wanted to look at them, So I showed them to her. he then went on and said â€Å"that they are expensive ,† but I also stated that they were for a good cause. In the past year of September, TOMS sold over one million pair of sh oes. His company is becoming more successful, and are growing bigger and bigger each day. Blake based the shoe style on the alpargata, a traditional Argentine shoe that farmers have worn for more than 100 years. The shoe is very lightweight and canvas slip-on with just a sliver of a rubber composite sole. It comes in multiple colors and prints for women, men and children. Toms comes out with a new style almost every six months.The shoe prices average from about $40-$100 dollars, They can be pricy but we also have to remember that its helping many kids without shoes, have shoes. The reason as to why Toms is so successful is, because they feel as if there making a difference in the world by helping the kids. They are satisfied and happy about themselves by helping the kids with no shoes. Toms has donated over 600 thousands paris of shoes to kids in 2010. The Power of Context Gladwell states, That the power of context, is no less more important than the two laws mentioned in the beginn ing of the book.Epidemics are sensitive in the time and places in which they occur. The Power of context is one that has to do with the environment we live in. We are the ones that get the word out. Like for instance the word of mouth epidemic is by a person telling them about it, then them telling someone else; or even when they see something that they like, People will ask you where you got the product, then tell his or her friends. Back In May of 2010, I was at a Romanian convention and I noticed a guy wearing them and wanted to find out what kind of shoes they were, because they looked very cool and comfy.I then went on found out where he got them, and bought two pairs, Now Most of the majority of the romanian community owns at least a pair of toms, if not even more. That explains why so many trends like skinny jeans, and or Jeans tucked in boots start. The environment has a lot to do with how epidemics occur, And that is how many trends come about. Just like in the book, The Ti pping Point there were two kids that were wearing hush puppies in East village, and some high fashioned stylists seen them. Later on they skyrocketed and sold over 600 thousands pairs.There is no other explanation more better than how the environment has many effects on how we look and or how we dress, the environment plays a big part on how products tip and how trends become more popular. Conclusion The reason why products tip are because the message itself had an impact, and or the environment where they were seen was noticed by big entrepreneurs. Why is it that many people like myself? are buying all into this, One thing is that it is contagious and the little things that make a change make a difference in the world.Trends and or diseases are also just as contagious. The co-founder of Toms travels to many different colleges to reach out to peers because he wants the new generations to be concerned and aware with different things. and helping others is one that everyone can do if they cant help by buying a pair they can help by not wearing shoes for 24 hours and that event that is called One Day Without Shoes, is targeted mostly to students because they are the ones with time and its a free advertising by getting his shoes known. and what more not doing it at colleges.Blake wants to raise up a better generations for our kids, and by helping and joining events just like this one is a great opportunity for many students to be aware and help make difference in the world. That is why they have an event every year, In Fact It is coming up shortly Sometime in April. The event mostly focuses on the awareness for people to realize that in many different parts of the country children are without shoes. Also to get others student to join and help if not by buying the shoes but by simply getting the word out.The One Day Without Shoes event allows student-led college chapters of TOMS to hold awareness events on campus (and do some free advertising for TOMS). The movemen t reaches out to students who want to help and be a part of a larger philanthropic movement, but can’t afford to give large amounts of money or commit an extended amount of time. People who are already passionate about helping children in need are the prime candidates to join the TOMS movement, so the student chapters and the organization reach out to these individuals through their  advertisements  and promotions. So having your world on your fingertips will be even more critical, and what better than your mobile phone for that! Our hero John McCabe is burdened with trekking an uneven, rocky, meandering path through the snow-capped woods a path as restrictive and uncomfortable as the harshest of Fords interiors. The mise-en-scà ¨ne elements Altman employs in this case serve to di stinguish McCabe Mrs. Miller from the classic western formula by providing a stark contrast in setting (and, by extension, tone). It is not a complete contrast, however. As film reviewer and Altman scholar, Gregory Lallone writes, Altmans interiors are just as suffocating, his untamed towns just as dangerous and ruled by greed, brutality, and chaos. It is simply a little warmer inside. Thus we may begin to analyze the lighting elements Altman employs in depicting McCabe Mrs. Millers interior locations as similarly untamed but significantly warmer than the surrounding, harsh outdoors. Altmans interiors are met with a profound use of low-key lighting to reflect this dim, foreboding, and even risquà © nature of the towns bars, businesses, and brothels. This is another clear departure from the traditional Hollywood string instruments (notably banjos, guitars, and fiddles), harmonicas, and bum-bum-BUMs that embellish dramatic moments in traditional western cinematography. As such, the ambient, diagetic soundtrack plays an especially central role in establishing tone and dramatic tension. During the final shootout, silence dominates so much of the audio track that every rupture of silence becomes vitally important. Each footstep, each crack of a wooden plank, and each breath McCabe takes might very well give away his position and result in his death. Thus we see that the intentional absence of non-diagetic sound can be just as effective as (if not more so than) its inclusion. Just as the frontier landscape is modulated, made somehow more truthful with the insertion of a bit of dirt and realism, the protagonist himself is similarly transformed.  While differing in many respects to the conventional western hero, John McCabe is actually not too far removed. His fight is largely the same defending what is rightfully his against outlaws and big business. He differs in that he lacks the toughness, the braggadocio, and the super-human courage of the Gary Coopers and Henry Fondas. Any shred of idealism and heroism McCabe claims to embody is counteracted by a narrow-sighted quest for profit, instances of clumsiness, and displays of outright cowardice. John McCabe is not a cowboy, a homesteader, a sheriff, or of some honorable profession; he owns a whorehouse. McCabes non-traditional characterization is further evidenced during the final showdown where the shootout is more accurately described as a slow and anxious game of cat-and-mouse. McCabe hops from one hid ing place to another knowing that winning a gunfight out in the open Gary Cooper-style is unrealistic. A true Western hero might denounce McCabes tactics as cheating and cowardly. He shoots two of the three gunmen in the back from a concealed hiding place and he overcomes the third by playing dead. Departing from the larger-than-life nature of classical western heroes, director Robert Altman injects a darker realism into his protagonist that reflects the revisionist nature of the film. By making particular, non-traditional use of various mise-en-scà ¨ne and sound elements in McCabe Mrs. Miller, director Robert Altman, refutes the conventional narrative pioneered by the classic Western that the American frontier was a sort of idyllic paradise. Ford depicted the frontier as the quintessential American arena where battles were fought and won by good men who, because of their very nature, triumph over the bad. Altmans western frontier is simply no more than a showcase of lawless capitalism and greedmen and women mercilessly arguing and fighting over profits like fleas over rotting flesh.
